Proof Of Vaccination Q & A

What is acceptable as proof of vaccination?
--CDC vaccine card or photo of vaccine card.
--Documented proof of vaccination from a medical record or vaccine provider.
--Proof of vaccination from another state or county.
--Printed certificate or digital record from apps.

Will I also be asked to show an ID?
Yes, we will cross reference the name on the vaccination to your student or personal ID, driver’s license or passport. Showing a fake vaccination card is a federal crime.

How is ‘Fully Vaccinated’ defined?
At least two weeks have passed since the final dose.

Why is this policy necessary?
We believe this is the safest way to offer a dance. Vaccinated individuals wish to safely return to something close to normal. We want to minimize the chance of spreading the virus to unvaccinated individuals.

Are masks still required?
Illinois remains under a mask mandate. Masks are recommended for everyone’s optimal safety. Staff will encourage but not insist that attendees wear a mask. Vaccinated people can still spread COVID-19. If you have a weakened immune system, it is wise to wear a mask while at the dance.

What Vaccines are accepted?
The following vaccines are accepted for verification having been authorized by the U.S. Food and Drug Administration (FDA) or World Health Organization (WHO): Pfizer, Moderna, Johnson & Johnson/Janssen (single dose), AstraZeneca, Sinopharm-BBIBP, Sinovac and Novavax.

Are negative covid tests, medical or religious exemptions acceptable as an alternative to vaccination?
No. There is no alternative to being fully vaccinated.

If I exit and reenter the event, do I have to show proof of vaccination again?
No. We will stamp everyone’s hand once they have been approved to enter. Show your stamp to reenter the dance.
